  3. Make sure the bank includes the “J” number and Project name on the check along with your name and address.
  4. The amount of the support can be:

i. a singular amount

ii. an amount taken out each month

  1. The total of your donation (should match the cheque)
  2. preferred project “J” number and project name
  3. The name and address to mail (or email) the receipt to,
